58 yr old housewife with

PATIENT HISTORY: Pt is a housewife and mother of 3 (2 sons, 1daughter). At the age of 34 one day while climbing stairs she felt a bit dizzy. So, went to Dr. and hypothyroidism was detected. In 2007, due to skin irritation and pus discharge from the lt. side of the anus while passing stool she went to Dr. and fistula in ano detected. After some temporary treatment for the same when problem still persisted Sx was recommended but pt. avoided it. In 2010, due to oophorectomy because of ovarian cyst/hysterctomy done. 5 yrs ago when she went for a ?normal health checkup; HTN and DM2 detected. At that time oneday while waking up from sleep in the morning everything started spinning and she felt dizzy. Went to R. who diagnosed it to be vertigo.Is not taking any medication for it but following Dr. advice (what advice?). When was the second attack. She is suffering from hallux varus deformity in both toes since when?. what she noticed first? She has walking and standing difficulty (very painful) so cannot stand or walk for long. She was adviced to take Gabapin nt but after taking it did not suite (what happened?). Since childhood she her stool is of loose consistency. If she eats roti in dinner she will have gastric issues. Any addiction?

Present history: Due to pain since when? below knee jt extending till ?toes, she was given Gabaneuron nt by family Physician. So, what happened? On 23.6.22, in her whole left side she started feeling a tingling sensation( went away?) and also chest pain on the left side which was migration towards back. Dr. said her heart is fine. On the day of doing Vit D3 test she was fasting till 12pm. Because of low serum Ca2+ she was given lumia 60k. On the day of the test, she vomited 5-6 times and diarrhea. Due to dehydration was admitted in hospital. Discharged after 2 days. Lt. side whole leg pain since when? Currently she wants a second opinion about whether to do Sx for fistula in ano and what? .

29 yr old male with c/o constipation, bleeding pr and gastric problems

  PATIENT HISTORY: Pt is a 29 yr old male working in a jewelry making shop. At the age of 7yrs, he underwent an operation for inguinal scrotal hernia. Since childhood he suffers from gastric problems. When in class 3-4, tooth extraction done due to dental caries. Pt at that time loved to eat sweets and chocolates. At the age of 11 yrs, in 2006, went to kerala to work in a jewelry making shop. His gastric problems increased (suspected etiology new kind of food, used to eat spicy foods, work very late night). He felt bloated, tenesmus, hard stool(3-4 times/day), constipation, flatulence, abd. pain.  Occasionally, few drops of raw blood used to come out with stool (2-3 days continuously, then again with a gap of 1-3 months).
